OB PATIENT CARE/SCRUB TECH, FAMILY MATERNITY UNIT - PART-TIME NIGHTS - 22379

This is a Part Time Night Shift Position

OB Patient Care/Scrub Tech:  Under supervision of the registered nurse, performs a variety of tasks and services to assist in the provision of care and comfort to patients; reports and records observations; contributes to the efficient functioning of the unit; all duties are performed in accordance with established policies, procedures, standards and guidelines.

To perform “scrub” technologist duties for scheduled and unscheduled cesarean sections, assist with the draping and utilization of instruments for a OB related surgical procedures.  Demonstrates knowledge in the basic principles of sterile technique and sterilization procedures. 

Qualifications

Education:

  • One of the following is required:
  1. Certified Nurse Assistant (CNA) 
  2. Minimum of two (2) years of relevant work experience performing the essential responsibilities of the nursing assistant role.
  • High-school graduate diploma or GED required. 

Licensure/Certification/Registry:

  • Maintains current CPR certification
  • CNA certification preferred

Other Knowledge/Skills/Abilities:

  • Successful completion of Nursing Team On-boarding, unit based orientation (including OR technician training program) and clinical competency validation within 60 days of hire. 

Responsibilities

I. Performs and documents nursing care measures to meet the physical needs of patients as directed by the registered nurse. Illustrative tasks and expectations include, but are not limited to, the following:

A. Provides hygiene and comfort measures, assuring privacy and preventing undue exposure:

  1. Provides or assists patient with bathing, oral hygiene, hair care, nail care, shaving, etc.
  2. Assures that linens are dry, wrinkle free and that patient is comfortable.
  3. Provides handwashing opportunity before and after meals, toileting, etc.

 

B. Activity:

  1. Assists with patient’s activities including ambulation, chair, dangling and repositioning; uses proper body mechanics, positioning techniques, and assistive devices.

C. Nutrition:

  1. Serves and collects food trays and nourishments.
  1. Records percentage of diet intake.
  1. Feeds or assists patients as necessary. Records intake.
  1. Reports deviations from normal.
  1. Clears IV and Tube Feeding Pumps at end of each shift and documents appropriately.

D. Elimination:

  1. Provides bed pan.
  1. Performs perineal care.
  1. Performs bladder catheter care and discontinues bladder catheter as directed.
  1. Performs ostomy care (ileostomy, colostomy, urostomy) including application of appropriate appliances.
  1. Records output. Reports deviations from normal.

E. Other related needs:

  1. Assists with post-mortem care.
  1. Assists with admission, transfer and discharge of patients.
  1. Promptly responds to patient call light or requests (usually in less than ten minutes).

II. Performs and documents monitoring procedures; observes and reports the patient response. Illustrative tasks and expectations include, but are not limited to, the following:

A. Obtains vital signs (B/P and TPR).

B. Obtains weight in kg (standing, chair, or bed scale) and reports deviation from previous weight.

C. Empties drainage collection devices.

D. Performs blood glucose monitoring.

E. Observes and reports unusual conditions and/or change in patient behavior or appearance.

III. Performs therapeutic measures/procedure, observes and reports changes, deviations from normal. Illustrative tasks and expectations include, but are not limited to, the following:

A. Application of heat; i.e., K-pad.

B. Application of cold; i.e., ice bags, sponge baths.

C. Therapeutic baths; i.e. Sitz, Aveeno.

D. Obtains routine non-sterile specimens.

E. Assists with patient preparation for surgical, therapeutic, and/or diagnostic procedures.

F. Oxygen therapy (sets up/maintains/discontinues) as directed.

G. Applies nonsterile dressing as directed.

H. Discontinues peripheral IV as directed.

I. Performs oral pharyngeal suctioning.

J. Preps skin and applies telemetry patches.

K. Records all interventions; observes and reports the patient response, changes, and/or deviations from normal parameters.

IV.Maintains safety and security of patient/family/self/others, observing safety precautions and promoting a sense of security and well-being. Illustrative tasks and expectations include, but are not limited to, the following:

A. Conducts purposeful hourly rounding as a team member, inquiring of the four P’s (Pain, Potty, Possessions and Positioning).

B. Follows standard precautions. Always washes hands between patients.

C. Reports equipment malfunctions.

D. Assists in keeping patient rooms/corridors free of clutter. Cleans fluid spills.

E . Uses proper body mechanics in moving, transferring and transporting patients.

F. Positions side rails appropriate to the patient’s condition.

G. Applies and maintains restraining devices as directed.

H. Interacts with patients/visitors/others in a courteous and respectful manner.

I. Fully explains procedures to patients/families prior to performing.

J. Assures patient privacy/rights/confidentiality.

K. Recognizes and reports patient concerns; offers constructive solutions.

V. Promotes efficient and effective functioning of the unit/hospital. Illustrative tasks and expectations include but are not limited to, the following:

A. Skilled in computer messaging, obtaining results.

B. Completes assigned aspects of unit inventory management.

C. Complies with established policies, procedures, standards and guidelines.

D. Uses designated lines of communication and authority.

E. Supports efforts to reduce cost.

F. Consistently utilizes time and resources effectively and efficiently.

G. Maintains cleanliness of unit, service area and equipment.

H. Cooperates with all members of the healthcare team.

I. Enthusiastically supports and participates in planned changes.

J. Accepts responsibility for accurate and timely completion of tasks as assigned.

K. Adheres to MHS Behavioral Standards.

L. Handles floor stock medications or medications delivered from Pharmacy directly or through the pneumatic tube system. Relocate these medications, in original containers, to designated secure storage locations or deliver directly to appropriate licensed staff.

 

Vi. Performs additional unit specific tasks/care elements for which they have received specific instruction and demonstrated competence.

 

Principle responsibilities for Scrub Tech role

 

1. Performs duties in capacity of “scrub” technologist as prescribed by hospital policies and procedures.

A. Sets room for a routine, uncomplicated procedure with supplies and equipment in 5-15 minutes.

 

B. Sets room for complicated or multi-procedure case requiring many special supplies in 30 minutes.

C. Assists circulator in checking equipment.

D. Assist circulator in obtaining supplies prior to bringing patient to room.

E. Has basic knowledge of instrumentation and its appropriate use.

F. Checks instrumentation regarding cleanliness, function, correct number of pieces, and completeness of tray.

G. Allows adequate time for preparation of supplies and counting (i.e., routine case 15 minutes, complicated case 30-40 minutes).

H. Has basic knowledge of surgical procedures.

I. Organizes and maintains a neat, clean mayo, instrument, and back tables.

J. Attempts reasonable anticipation of sequence of instrumentation.

K. Properly identifies medication.

L. Handles floor stock medications or medications delivered from Pharmacy directly or through the pneumatic tube system. Relocate these medications, in original containers, to designated secure storage locations or deliver directly to appropriate licensed staff.

M. Keeps track of sponges, needles, and instruments during procedure at all times.

N. Relays accurate identification of specimens and cultures.

O. Organizes and assists in end-of-case clean up.

P. Separates sharp and delicate instruments during clean up.

Q. Assists in pulling cases.

 

II. Assists in maintenance of safe environment for patients and staff.

A. Has knowledge and applies principles of good aseptic practice (i.e., appropriate scrub length, OR attire, proper gowning, gloving, and draping methods).

B.Checks sterile supplies for proper packaging and dating.

C. Actively assists in recognizing and reporting breaks in sterile technique.

D. Requests assistance of Sterile Processing personnel in preparation of instruments for immediate reuse.

E. Follows protocol for universal blood and body fluid precautions at all times.

F. Prevents loss of instruments by thorough search of drapes before they are discarded.

III, Assists in the orderly function of the OR.

A. Demonstrates flexibility in a team effort (i.e., takes required supplies from circulator when offered).

B. Checks room for and put away excess supplies and equipment.

C. Takes initiative and seeks appropriate opportunities to update and maintain surgical skills (i.e., requests certain cases and attends inservices).

D. Assists in checking to-follow cases for completeness and accuracy.

E. Assists in orientation of new personnel.

F. Actively participates in checking and maintaining assignment by participating as a permanent team member.

G. Participates in updating surgeon’s preference list.

H. Reports to assigned room on time.

I. Understands and follows disaster plan.

J. Follows hospital/unit policies, procedures, and behavioral expectations.

K.. Uses designated lines of communication and authority.

L. Willingly assists co-workers to facilitate patient care.

IV. Initiates appropriate intraoperative behaviors unique to first assistant, if trained to do so.

A. Provides hemostasis to reduce potential for post-operative infection and potential for fluid volume deficit by clamping and/or ligating vessels, using electrosurgical cautery, applying direct pressure, applying chemical agents that promote coagulation, and other means as directed by surgeon.

B. Provides exposure of the operative site through the appropriate use of retractors and other instruments and through suctioning and sponging.

C. Uses surgical instrumentation skillfully in ways consistent with their design and purpose as directed by the surgeon.

D. Handles all variety of tissue appropriately to reduce potential for injury or compromised blood supply.

V. Performs other related work as required or requested.
