Decatur, ILProvides a range of social work / services to patients served by the MHS and Hospice Care of Illinois. Assist the patient, family, physician and other team members in dealing with the significant psycho-social and emotional factors related to the illness in the home setting. Participates in the development and follow-up of patient’s plan of treatment; documents progress notes and utilizes appropriate Memorial Health Systems and community resources appropriate to the case.
Qualifications
Requirements:
- Master Degree in Social Work from a School of Social Work accredited by the Council on Social Work Education.
- Illinois Licensed Social Worker required. Licensed Clinical Social Worker preferred.
- One (1) year experience in Social Work, preferably in the field of Medical Social Work. Clinical experience appropriate to the counseling and casework needs of the assigned caseload.
- Will accept a Memorial Home Services’ car for business purposes only as deemed necessary by geography or mileage. Must possess a valid IL driver license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required).
- Ability to travel as needed to support caseload and required to participate in on-call rotation.
Responsibilities
Provides Social Work services to all programs of the agency in collaboration with the healthcare team and order from the physician.
Provides Social Work intervention and follow-up of assigned cases in accordance with standards of Social Service practice and guidelines of policies and procedures of the Agency.
Develops psychosocial plan appropriate to patient’s age and developmental stage and collaborates effectively with multidisciplinary team to achieve objectives of the stay. Assumes ultimate responsibility for completion of all documentation requirements on assigned patients.
Evaluates and re-evaluates the patient’s and family’s psychosocial needs.
Promotes efficient and effective functioning of the agency in the interest of patient care.
Acts as a patient advocate and provide education and assistance regarding social/economic factors and treatment plans to promote acceptance and cooperation with psychosocial demands of illness.
Coordinates discharge planning services.
Participates in the on-going as well as project planning activities within the organization for development of self and other staff.
Refers patients and families to community agencies with appropriate follow-up.
Abides by the National Association of Social Work (NASW) Code of Ethics and adheres to Memorial’s vision, mission, statement of values and behavioral standards.
