Job Description Summary:

As a Physical Therapist, you will be responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines. Bachelor’s degree in Physical Therapy or entry level equivalent is required. Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state. Current CPR card. Job Description:

Education: - Graduate of an approved school of Physical Therapy. - Basic knowledge in physical therapy practices in accordance with the P.T. Practice Act. Licensure/Certification/Registry: - Illinois eligible or licensed Physical Therapist. - CPR certification is required. Other Knowledge/Skills/Abilities: - Universal Precautions Category I. - Must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance.

Pay Transparency

$37.43 - $58.02

Additional compensation in the form(s) of participation in our annual incentive program are dependent on role. Pay is based on factors such as work experience, education, certification(s), training, skills, and competencies related to the role.
